Birth Certificates
The City of Farmington Hills can provide certified copies of
birth certificates for all births that occurred after July 1973 at
Botsford Hospital or home births within the City of Farmington Hills.
Special provisions are as follows:
IF BORN PRIOR TO JULY, 1973, birth certificates may be obtained
from the Oakland County Vital Statistics Department, (248) 858-0571 (as
long as the mother was married at the time of birth), or the State of
Michigan, Vital Records Department, (517) 335-8666 (all births).
IF BORN BETWEEN JULY, 1973 AND SEPTEMBER 30, 1978, a copy may
be obtained at the City of Farmington Hills ONLY if the mother was
married at the time of the birth. If the mother was not married, the
certificate may be obtained at either Oakland County or from the State
of Michigan (numbers above).
The City of Farmington Hills has copies of all births within
the City after October, 1978.
Who can get a copy of a birth certificate?
A mother or father only (whose name appears on the certificate)
may obtain a copy of their child's birth record with proper (picture)
identification. Any person age 15 years or over may obtain a copy of
their own birth record with proper picture identification.
PROPER IDENTIFICATION includes a valid driver's license,
state-issued identification, passport, or school identification. ID must
provide your signature and picture.
COSTS: $15 – first copy; $5 for each additional copy ordered at
the same time.

Death Certificates
Certified copies of death certificates may be obtained at the
City Clerk's Office for any death occurring in the City of Farmington
Hills after July, 1973. For any death prior to July, 1973, copies may be
obtained at either Oakland County or the State of Michigan (phone
numbers referenced above).
COST: $15 – first copy; $5 for each additional copy ordered at
the same time.

To obtain certified copies of birth or death records, you may either
apply by mail using the applications available through this website, or
you may visit the City Clerk's Office at the Farmington Hills City Hall
any weekday from 8:30 AM – 4:30 PM.
