Passport Acceptance Facility
The Farmington Hills City Clerk’s Office is now a passport acceptance
facility. We accept applications and forward them to Chicago for
processing. WE DO NOT ISSUE PASSPORTS.
U.S. Passport “Cards”
The U.S. Department of State as of February 1, 2008, will be issuing
U.S. Passport Cards that are valid for travel by land and sea crossing
between the U.S. and Canada, Mexico, Bermuda and the Caribbean ONLY.
They are wallet size cards and are valid for 10 years for adults (16 and
over) and 5 years for minors (15 and under). If traveling by air, you
would still require a U.S. Passport Book. Appropriate fees are listed
below for both the Passport Card and Passport Book. You may apply for
either or both depending on your needs.
Basic Requirements to Obtain a U.S. Passport
(Please note that all documents must be original documents and must
be relinquished at the time of applying. If you are applying with the
City of Farmington Hills Clerks Office, you may not use a birth record
that was issued by our office. You must use a State issued birth record.
All documents will be returned with your passport.)
Proof of U.S. Citizenship
a. Previous U. S. Passports.
b. Certified Birth Certificate (U.S. born) – no copies.
c. Naturalization Certificate.
e. Report of Birth Abroad.
Proof of Identity
a. Driver’s License (not temporary or learner’s permit).
b. U.S. Passport (not expired).
d. Official Military Identification Card.
Two Photographs
- Must be 2” x 2” head and shoulders (not too close up where just
head shows).
- No head covering is allowed unless used for religious beliefs
(i.e. turbans, etc). You must put in writing that you wear the
head covering at all times for religious purposes. Muslim women
may not wear a veil covering half their face.
- Nuns may wear religious clothing.
- No uniforms in pictures (police, fire, etc). This is for
protection of applicant.
- No dark glasses permitted – must be able to see eyes.
- Baby pictures – should try to get with eyes open.
- Digital photos okay if on Kodak paper.
APPROPRIATE FEES (effective February
1, 2008)
U.S. Passport Book:
- $75 Passport Processing Fee (check payable to the U.S. Department
of State) – 16 years of age or older (10-year passport).
- $60 Passport Processing Fee (check payable to the U.S. Department
of State) – 15 years of age or younger (5-year passport).
- $25 Execution Fee for each application** (check payable to the
City of Farmington Hills).
- EXPEDITED PASSPORTS – Additional $60 plus overnight postage both
ways is suggested ($16.25 each way)
U.S. Passport Card:

of State) – 16 years of age or older (10-year passport).
- $60 Passport Processing Fee (check payable to the U.S. Department

City of Farmington Hills).
- EXPEDITED SERVICE FOR U.S. PASSPORT CARDS IS NOT AVAILABLE AT THIS
TIME.
Expect to receive your passport in approximately 6-8 weeks.
**All applicants must pay a $25 execution fee when applying at a
designated acceptance facility. This fee is collected by the City
Clerk’s Office as a designated facility and is the same at all
acceptance facilities.
- Checks should be made payable to US Department of State for
passport book or passport card fees plus any expediting fee and return
postage fees if applicable. Check or money order should have
applicant's name on it and date of birth.
- A separate check should be made payable to the City of
Farmington Hills in the amount of $25 for execution fee plus any
overnight postage fees related to expedited passports if applicable.
